Dr. Mary Ann Bough, DC is a licensed Chiropractor in Indiana serving the Bloomington community. Dr. Mary Ann Bough, DC graduated from Sherman College of Chiropractic in Spartanburg, SC in 2001. She graduated Magna Cum Laude with Clinical Excellence Distinction. She also received her undergraduate degree from Indiana University in 1996 with a Bachelor of General Studies: Science and Math Concentration and a Psychology Minor. Dr. Mary Ann Bough, DC is certified in computerized adjusting and analysis of the spine with the PULSTAR System. Dr. Mary Ann Bough, DC has been freeing people from pain and improving overall health for 23 plus years.  As a Chiropractor, Dr. Bough, DC is committed to promoting optimal health and well being of patients. She uses a 'whole person approach'. This approach to wellness means looking for underlying causes of any disturbance or disruption (which may or may not be causing symptoms at the time) and makes whatever interventions and lifestyle adjustments that would optimize the conditions for normal function. Using this unique approach, Dr. Bough is able to help you to accelerate and/or maintain your journey to good health.

Unlike conventional medicine, which focuses on attempting to treat disease once it occurs, Hoosier Family Chiropractic, Inc. emphasizes improving your health in an effort to reduce the risk of pain and illness in the first place. What to Expect on Your First Visit

Patient Forms: Once we set up your initial appointment you will receive a link via email for our online new patient paperwork. All paperwork can be completed online and does not need to be printed. Upon entering our office, our receptionist will welcome you as a member of our family. If you were unable to complete the paperwork online, we will then request that you complete our new patient forms in our office.

Examination: Next, one of our Chiropractic Assistants will perform the beginning portion of an initial examination. This includes a Posture Analysis, Range of Motion Screening, Nerve and Muscle Testing, and Heart Rate Variability.  Dr. Mary Ann Bough, DC will then do additional testing as needed.

ConsultationNext, you will have a consultation with Dr. Mary Ann Bough to discuss your health-related problems, concerns, and potential treatment options and goals. 
What to Expect on Your First Visit

Treatment: You will receive your first adjustment on your initial visit.  Treatment is interactive and customized, so you can have full participation in your care.

Plan of care: Prior to leaving, the doctor will provide you with recommendations to incorporate outside of treatment. If you desire, our wellness team will work with you also to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized plan. The purpose of our plan of care is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.

X-Ray Studies: X-rays can help us identify and/or rule out specific pathology and help guide appropriate treatment. X-rays are performed when deemed clinically necessary based on that individual's case.

Report of Findings: Once the information is collected and examinations are performed, Dr. Mary Ann Bough, DC will give you a detailed report of all your exam findings and answer any questions. She will discuss recommendations and notify you if your condition requires additional care with other providers.

I understand that I have certain rights to privacy regarding my protected health information. These rights are given to me under the Health Insurance Portability and Accountability Act (HIPAA). I understand that by signing this consent I authorize you to use and disclose my protected health information to carry out:
  • Treatment (including direct or indirect treatment by other healthcare providers involved in my treatment);
  • Obtaining payment from third party payers (e.g. my insurance company);
  • The day-to-day healthcare operations of your practice.
I have also been informed of and given the right to review and secure a copy of your Notice of Privacy Practices, which contains a more complete description of the uses and disclosures of my protected health information and my rights under HIPAA. I understand that you reserve the right to change the terms of this notice from time to time and that I may contact you at any time to obtain the most current copy of this notice. I understand that I have the right to request restrictions on how my protected health information is used and disclosed to carry out treatment, payment, and health care operations, but that you are not required to agree to these requested restrictions. However, if you do agree, you are then bound to comply with this restriction. I understand that I may revoke this consent, in writing, at any time. However, any use or disclosure that occurred prior to the date I revoke this consent is not affected.

No Surprises Act

*You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.* Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
